NDIS Support Services
Nursing
Our nursing support is here to make it easier to manage your health at home. We provide personalised nursing care, whether it’s health monitoring, or support with complex clinical needs. Our goal is to help you feel comfortable and confident about your health, and to minimise visits to hospital.
Assistance with regular health check-ups and keeping track of your mental and physical wellbeing.
Provide education around medication, side effects and assist with management of both.
Support Coordination
Navigating the NDIS can feel overwhelming, but that’s where we come in. Our support coordination helps you make sense of your plan and connect with the right services. We’ll be with you every step of the way, helping you set up supports, solve any issues, and make sure you’re getting the most out of your NDIS plan.
We’ll work with you to find the right support providers for your needs.
Help with organising appointments and making sure everything is running smoothly.
Support with reviewing your NDIS plan to see what’s working and what’s not.
Community Access
Getting out and about can be a great way to boost your mood and feel connected. Our community access support is all about helping you take part in activities you enjoy, build social connections, and explore new places. Whether it’s catching up with friends or joining in on local events, we’re here to support you in feeling part of your community.
We can accompany you to social groups or community events to build your confidence.
Support with using public transport, so you can get around more independently.
Help with organising fun outings like a trip to the local markets or a café visit.
Therapeutic Supports
Therapeutic supports are all about helping you build strategies to feel your best. This can include mental health therapies, counselling, or support to improve your physical and emotional wellbeing. We’ll work with you to identify your goals and offer practical, person-centred therapy to help you overcome challenges and make positive changes in your life.
Access to therapeutic counselling to help manage stress, anxiety, or other mental health concerns.
Support to build coping strategies that promote emotional resilience and self-care.
Assistance with physical therapies like exercise programs to improve your overall wellbeing.
